چکیده

AIM The purpose of this prospective clinical study was to examine the short- and long-term psychosocial outcomes of a consecutive series of patients who underwent extratemporal lobe resection due to medically-refractory epilepsy. MATERIALS AND METHODS The sample consisted of 23 consecutive patients and all patients completed a questionnaire assessing especially psychosocial outcome 6 months and 2 years after surgery. Results obtained at short- and long-term follow-ups were compared to baseline. Furthermore, the impact of seizure freedom on the psychosocial outcome was sought. RESULTS The results suggested that, psychosocial outcome was improved after surgery compared to preoperative status regardless of seizure status. At long-term follow-up, significant improvements were found in social and psychological variables (p < 0.05). Levels of side effects from medication were high at long-term compared to baseline (p = 0.003). Seizure free patients showed better psychosocial outcome than those who had seizure during the postoperative period, however; only the "impact of epilepsy" scale showed significant improvement at 6 months after surgery (p = 0.02). CONCLUSION These results provide evidence that surgery caused appreciable improvements in psychosocial well-being, however; seizure freedom is not key to improving the psychosocial life of patients who have undergone extratemporal lobe epilepsy surgery.

The early identification of candidates for epilepsy surgery.

The effectiveness of resective surgery for the treatment of carefully selected patients with medically intractable, localization-related epilepsy is clear. Seizure-free rates following temporal lobectomy are consistently 65% to 70% in adults and 68% to 78% in children. Extratemporal resections less commonly lead to a seizure-free outcome, although one recent childhood series reported a seizure-...

The long-term outcome of adult epilepsy surgery, patterns of seizure remission, and relapse: a cohort study

BACKGROUND Surgery is increasingly used as treatment for refractory focal epilepsy; however, few rigorous reports of long-term outcome exist. We did this study to identify long-term outcome of epilepsy surgery in adults by establishing patterns of seizure remission and relapse after surgery. METHODS We report long-term outcome of surgery for epilepsy in 615 adults (497 anterior temporal resec...

Long-term outcome and predictors of resective surgery prognosis in patients with refractory extratemporal epilepsy

PURPOSE We analyzed the long-term postoperative outcome and possible predictive factors of the outcome in surgically treated patients with refractory extratemporal epilepsy. METHODS We retrospectively analyzed 73 patients who had undergone resective surgery at the Epilepsy Center Brno between 1995 and 2010 and who had reached at least 1 year outcome after the surgery. The average age at surge...

Sleep-Related Seizures in Refractory Focal Epilepsy: Electroclinical Findings and Surgical Outcome

Background: Sleep Seizures (SSs) generally occur in refractory focal epilepsy, but their detailed characteristics and circadian patterns are still controversial. The effect of SSs on epilepsy surgery outcome has been addressed in few studies without definitive prognostic value.   Objectives: This study investigated the characteristics of SSs and their prognosis in refractory focal epilepsy. M...
